We close that gap by holding the delivery accountability on the client side throughout. We write the requirements with the business users who will live with the system. We run the UAT with the people whose processes the system is replacing. We manage the cutover with the teams who will operate the system post-go-live. We do not hand over and walk out at technical acceptance.

We have also rescued programmes that were running significantly late by restructuring workstreams, replacing informal handover processes with formal dependency management, and putting named accountability on every critical path decision. If a programme is in trouble, the first question we ask is not "what went wrong" but "who is accountable for what, and does the programme structure support that accountability?"
